I need to make a benefit change

If you are a regular employee of Deiter Bros., working 30 or more hours per week, you are eligible to participate in the Medical plan. If you are a regular full-time employee working 40 or more hours per week, you are eligible to participate in all benefits. Benefits begin on the first of the month following 30 days from your date of hire. Part-time employees working less than 30 hours per week and seasonal employees are not eligible to participate.

If you don’t enroll by the date when your benefits will go into effect, you will not have the opportunity to do so until the next open enrollment period, unless you have a Qualifying Life Event.

Eligible dependents include:

Your legal spouse.
Your children up to age 26. Only those dependents who meet the eligibility requirements can enroll for coverage.

Employees who choose not to enroll in the Deiter Bros. medical plan may be eligible for a monthly opt-out incentive. This program is intended to provide a consistent benefit for employees who already have health coverage elsewhere and do not need coverage under the company plan.

Monthly Opt-Out Incentive Amounts

$400 per month for employees who waive individual medical coverage.
$600 per month for employees who waive coverage for themselves and eligible dependents. Proof of alternative minimum essential coverage is required, such as Medicaid, CHIP, coverage through another employer-sponsored health plan, or Medicare Part A.

Employees are encouraged to carefully evaluate their healthcare needs and choose the coverage option that is best for themselves and their families.
